Lab / Site / Org

An occurrence report should have a site (or laboratory/organization) associated with it. Individual sites are associated with specific facilities.

To specify values for this field, first select it on the Specify New Database Search Criteria page. Then click the Refine Search and Specify Values command button. Select one or more values using the Lab / Site / Org selection box shown in the image below. Note that OR logic is always applied when there are multiple selections within a selection box.

Last Modification Date

This is the date of the last modification to the occurrence report.

To specify values for this field, first select it from the Specify New Database Search Criteria page. Then click the Refine Search and Specify Values command button. Specify the date using the Last Modification selection boxes and check boxes, shown in the image below. To select a date range, click the Range checkbox, then click the < or > check boxes. To select a specific date, click the = check box and select the year, month, day, hour and minute of the last modification. Note that you may specify one or more check box. In the example below, the user is searching for all reports that were last modified any day of March 2003.

Latest Update Report Date/Time

This is the date and time the most recent Update Report was transmitted to the centralized computer. (If only one Update Report has been transmitted, then this date is the same as the Initial Update Report Date/Time.)

To specify values for this field, first select it from the Specify New Database Search Criteria page. Then click the Refine Search and Specify Values command button. Specify the date using the Latest Update Report selection boxes and check boxes, shown in the image below. To select a date range, click the Range checkbox, then click the < or > check boxes, indicating that you wish to search for all values less than or greater than a specified date. To select a specific date, click the = check box and select the year, month, day, hour and minute of the last modification. Note that you may specify one or more check box. In the example below, the user is searching for all reports less than January 30, 2001.

Lesson(s) Learned

This narrative field includes any lesson(s) learned from the occurrence that could be of importance to other facility operators or that should be addressed in personnel training or facility procedures. This field is required for all Final Occurrence Reports.

To specify values for this field, first highlight it on the Specify New Database Search Criteria page. Then click the Refine Search and Specify Values command button. Enter search text into the Lesson(s) Learned edit box shown in the image below. Within the edit box enter single words or words/phrases, combined with text operators. What you specify in this edit box will be used to search the Lesson(s) Learned text. In the example below, the user is searching for all reports that contain both the words "revise" and "procedure" in the Lessons Learned field.
